External Scholarship Program Application

The Trilogy Health Services Foundation (The Foundation) External Scholarship Program is available to current high school seniors or rising collegiate freshmen pursuing a health-related field and wanting to work with the geriatric population. The Foundation will grant two $2,500 scholarships per state, in which Trilogy operates Health Services campuses (currently KY, IN, MI, OH,WI) , annually for four years. The application will be open from January 1st to March 15th.

Please see below for the complete program requirements:

Applicants must live in Kentucky, Ohio, Indiana, Michigan, or Wisconsin. Applicants do not have to attend college in the state they reside in.
GPA requirement of 3.5 or above.
Applicants must be in good standing with their university to receive reoccurring scholarship funds.
Applicants must provide an up-to-date resume and transcript each year to be reconsidered.
