Temporary Nurse Aide Application 



Please read the following information before completing this application:

This application is to apply for the Temporary Nurse Aide competency, available during the Covid-19 outbreak by special waiver from CMS. Completion of the training (provided by an education partner) and the Competency Validation (completed with a Trilogy Nurse) will qualify you for $.50 premium pay and to be able to do the job duties of the CRCA until the conclusion of the Covid-19 outbreak. This application is for the competency only. Please speak with your DHS on how to get enrolled in the training from one of our education partners. 

Please note to receive the premium pay you must be in one of the following certifications, and not currently hold a State Tested Nurse Aide certification:

  • Life Enrichment Associate

  • Dining Services Assistant

  • Environmental Services Asst

  • Floor Technician

  • Guest Relations

  • Medication Technician

  • Resident Care Associate

  • Social Services Assistant

  • Staff Development Non Nurse

  • Transportation Associate
